Project Forms

Section 2: APR

This is the ONLY project report form to be used.  Please print one out for each project, and fill it in accordingly.  PROJECT SPECIFIC REPORT FORMS WILL NO LONGER BE USED!  Please use this to complete your Community Service APR, and any projects you plan on exhibiting at the fair or would like to be judged at the record book review.

Leadership Forms

Section 3: Jr. Teen Leader Report

This is a supplemental form to a project that you are enrolled in, in which you and the Project Leader have agreed upon this youth leadership position.  

A Jr. Leader is someone in grades 6-8th, between the ages of 11-13 by December 31st of the program year.

A Teen Leader is someone in 9th grade or 14 years old by December 31st of the program year.
